New York State allows patients direct access to PT services for 10 visits or 30 days without a physical referral.
By choosing direct access, you can save both time and money, as this eliminates the traditional process of waiting for an appointment and requesting a referral.
A referral might eventually be required to continue PT or for insurance claims, but you will not need it to start your rehab process.

Forward Physio is an out-of-network provider with all insurances.
Dependent on your insurance plan and out-of-network deductibles, a decision can be made whether to bill out-of-network insurance or if self-pay is a more viable option. Our medical biller will verify insurance and advice further.
Initial evaluation (60 minutes): $250
Follow ups (50 minutes): $180
Pediatric Travel Fee Policy: Services are provided within a 3-mile radius of our base at Queens Plaza N, Long Island City, Queens at the standard rate. Locations beyond this radius (e.g., past Ditmars in Astoria, Jackson Heights into Forest Hills, or McCarren Park in Williamsburg) incur an extra $15 travel fee per session, calculated via straight-line distance. This ensures fair compensation for concierge/in-home visits while supporting your child’s early intervention needs.

“In‑network” means the physical therapist has a contract with your insurance company. These providers charge lower, pre‑agreed rates, so your copay, coinsurance, and overall cost are usually lower.
“Out‑of‑network” means the therapist does not have a contract with your insurance, usually due to low reimbursement rates. They may bill at their regular rate, and depending on your insurance plan, it might add up to be the same or just slightly more than in network for more specialized care.
How going out‑of‑network might benefit you:
• You have more freedom to choose the therapist or clinic that best fits your needs.
• You may access specialized or higher‑intensity care that isn’t available from in‑network providers.
• You might get more flexible scheduling, longer sessions, or a specific treatment style that matches your recovery goals.
Keep in mind that if your plan does not cover out‑of‑network services (common with HMOs), you may be responsible for the full cost. Please contact us or your insurance carrier if you’d like help understanding your specific benefits.

The answer is, both! I am able to treat both populations as I did not choose one specialty over the other. I have worked with pediatric populations for more than 7 years, and continuing to keep my knowledge on treating athletic injuries (running, olympic weightlifting, etc) via clinical education courses that are approved by NYS.

JILLIAN LYE
PT, DPT, Cert-CMFA
Born and raised in Singapore, Jill’s passion for sports and rehabilitation began early as a representative of Singapore’s Junior Squash Team. She earned her Bachelor of Physiotherapy from Curtin University in Perth, Australia, and her Doctorate in Physical Therapy from the University of Montana. Having practiced across three countries, Jill brings a diverse clinical background in orthopedic and sports rehabilitation, as well as pediatric care for children as young as two months old.
Jill’s approach is grounded in individualized, evidence-based care that empowers patients through education and active participation in their recovery. She is an avid endurance athlete and CrossFit participant, holding certifications from the Institute of Clinical Excellence in Clinical Management of the Fitness Athlete and Rehabilitation of the Injured Runner.
In addition to her sports and orthopedic focus, Jill is an experienced pediatric physical therapist with a special interest in early motor development. Since 2017, she has worked with infants and children to support motor milestones and improve functional skills through therapeutic and purposeful play—partnering closely with parents to foster confidence and growth in their child’s development.
